You will need to bring a prescription from your provider. Please reach out to me concerning the need for updated forms from your doctor along with insurance cards. Elite Medical Supply accepts Medicare B, Medicaid, and most major insurances.

We will have a Certified Pedorthist there to help measure and insure a proper fit. There will be no copay required.
